Evaluating resampling methods and structured features to improve fall incident report identification by the severity level

ABSTRACT

OBJECTIVE: This study aims to improve the classification of the fall incident severity level by considering data imbalance issues and structured features through machine learning.

MATERIALS AND METHODS: We present an incident report classification (IRC) framework to classify the in-hospital fall incident severity level by addressing the imbalanced class problem and incorporating structured attributes. After text preprocessing, bag-of-words features, structured text features, and structured clinical features were extracted from the reports. Next, resampling techniques were incorporated into the training process. Machine learning algorithms were used to build classification models. IRC systems were trained, validated, and tested using a repeated and randomly stratified shuffle-split cross-validation method. Finally, we evaluated the system performance using the F1-measure, precision, and recall over 15 stratified test sets.

RESULTS: The experimental results demonstrated that the classification system setting considering both data imbalance issues and structured features outperformed the other system settings (with a mean macro-averaged F1-measure of 0.733). Considering the structured features and resampling techniques, this classification system setting significantly improved the mean F1-measure for the rare class by 30.88% (P value < .001) and the mean macro-averaged F1-measure by 8.26% from the baseline system setting (P value < .001). In general, the classification system employing the random forest algorithm and random oversampling method outperformed the others.

CONCLUSIONS: Structured features provide essential information for categorizing the fall incident severity level. Resampling methods help rebalance the class distribution of the original incident report data, which improves the performance of machine learning models. The IRC framework presented in this study effectively automates the identification of fall incident reports by the severity level.

Assessment of in vivo bone microarchitecture changes in an anti-TNFα treated psoriatic arthritic patient

ABSTRACT

OBJECTIVE: Psoriatic arthritis (PsA) is an inflammatory rheumatic disease, mediated in part by TNFα and associated with bone loss. Anti-TNFα treatment should inhibit this phenomenon and reduce the systemic bone loss. Ultra-high field MRI (UHF MRI) may be used to quantify bone microarchitecture (BM) in-vivo. In this study, we quantified BM using UHF MRI in a PsA patient and followed up the changes related to anti-TNFα treatment.

SUBJECTS AND METHODS: A non-treated PsA patient with knee arthritis and 7 gender-matched controls were scanned using a gradient re-echo sequence at UHF MRI. After a year of Adalimumab treatment, the patient underwent a second UHF MRI. A PET-FNa imaging was performed before and after treatment to identify and localize the abnormal metabolic areas. BM was characterized using typical morphological parameters quantified in 32 regions of interest (ROIs) located in the patella, proximal tibia, and distal femur.

RESULTS: Before treatment, the BM parameters were statistically different from controls in 24/32 ROIs with differences reaching up to 38%. After treatment, BM parameters were normalized for 15 out of 24 ROIs. The hypermetabolic areas disclosed by PET-FNa before the treatment partly resumed after the treatment.

CONCLUSION: Thanks to UHF MRI, we quantified in vivo BM anomalies in a PsA patient and we illustrated a major reversion after one year of treatment. Moreover, BM results highlighted that the abnormalities were not only localized in hypermetabolic regions identified by PET-FNa, suggesting that the bone loss was global and not related to inflammation.

Effects of insertion torque on the structure of dental implants with different connections: Experimental pilot study in vitro

ABSTRACT

OBJECTIVE: During the insertion of dental implants in the bone tissue, different torque values can be applied. However, the high applied torque can cause damage to the implant connection. Our study sought to evaluate, by measuring the angle of rotation of the insertion drive and, later microscopic observation, possible changes in the structure of implants of different diameters with 3 different types of connections after the application of 4 different torque intensities.

MATERIALS AND METHODS: Three hundred tapered dental implants and three hundred insertion drivers were used in the present study. Implants of 3.5 and 4 mm in diameter with 3 connection models were tested: external hexagon (EH), internal hexagon (IH) and Morse taper (MT). Then, sis groups were performed: EH3 group, EH4 group, IH3 group, IH4 group, MT3 group and MT4 group. The samples were submitted to the torque/torsion force at 4 intensities (n = 10 samples per group and intensity): 60, 80, 100 and 120 Ncm. The turning angle of the insertion driver was measured in each test. In addition, in 10 samples from each group, the maximum torque value supported by each implant model was measured. After the tests, all samples were inspected microscopically to describe the observed changes.

RESULTS: The maximum torque supported by the different implant models showed statistically significant difference (p < 0.0001). The values of the measured angles showed statistically significant differences between the torque values applied within each group (p < 0.001) and between groups with the same torque value (p < 0.001).

CONCLUSIONS: Within the limitations of the present study in vitro, the results showed that high torque values cause mechanical damage to the implants.

Grains on the brain: A survey of dog owner purchasing habits related to grain-free dry dog foods

ABSTRACT

Grain-free pet food options abound in the pet food market today, representing more than 40% of available dry dog foods in the United States. There is currently a dearth of information about the factors that contribute to a dog owner’s choice of a grain-free dry dog food and if those factors are similar among countries. Therefore, the primary objective of the current survey was to identify the variables that are predictive of a dog owner’s choice of a grain-free dry food across North America (Canada and the United States) and Europe (France, the United Kingdom and Germany). The survey consisted of 69 questions, took less than 15 minutes to complete and was distributed virtually via Qualtrics (Qualtrics XM, Utah, USA). A total of 3,298 responses were collected, equally distributed between countries. Multinomial logistic regression was performed in SPSS Statistics (Version 26, IBM Corp, North Castle, New York, USA). Male respondents, people from France, people who ranked the importance of ingredients in a pet food in the lower quartiles and people who do not rotate their dog’s diet to provide variety were less likely to select ‘no grain’ when choosing a pet food. In contrast, people who believe that their dog has a food allergy, follow more than 5 specific dietary routines in their own diet, do not try to include grains in their own diet, get their information about pet food from online resources or pet store staff and look for specific claims on pet food (such as ‘no fillers’), were all more likely to select ‘no grain’ when choosing a pet food. This survey provides insight into the similarities and differences in decision making among dog owners in North America and Europe and should be considered when exploring the effects of grain-free dog foods on canine health and well-being.

Functional impairment-based segmentation of anterior cingulate cortex in depression and its relationship with treatment effects

ABSTRACT

In major depressive disorder (MDD), the anterior cingulate cortex (ACC) is widely related to depression impairment and antidepressant treatment response. The multiplicity of ACC subdivisions calls for a fine-grained investigation of their functional impairment and recovery profiles. We recorded resting state fMRI signals from 59 MDD patients twice before and after 12-week antidepressant treatment, as well as 59 healthy controls (HCs). With functional connectivity (FC) between each ACC voxel and four regions of interests (bilateral dorsolateral prefrontal cortex [DLPFC] and amygdalae), subdivisions with variable impairment were identified based on groups’ dissimilarity values between MDD patients before treatment and HC. The ACC was subdivided into three impairment subdivisions named as MedialACC, DistalACC, and LateralACC according to their dominant locations. Furthermore, the impairment pattern and the recovery pattern were measured based on group statistical analyses. DistalACC impaired more on its FC with left DLPFC, whereas LateralACC showed more serious impairment on its FC with bilateral amygdalae. After treatment, FCs between DistalACC and left DLPFC, and between LateralACC and right amygdala were normalized while impaired FC between LateralACC and left amygdala kept dysfunctional. Subsequently, FC between DistalACC and left DLPFC might contribute to clinical outcome prediction. Our approach could provide an insight into how the ACC was impaired in depression and partly restored after antidepressant treatment, from the perspective of the interaction between ACC subregions and critical frontal and subcortical regions.

Ethnocultural and socioeconomic disparities in exposure to residential greenness within urban Canada

ABSTRACT

BACKGROUND: Residential greenness has been associated with health benefits, such as lower risk of mortality, cardiovascular disease, obesity, adverse birth outcomes and asthma and better psychological health. However, the variation in greenness across socioeconomic and demographic characteristics in urban areas of Canada has not been well documented.

DATA AND METHODS: Respondents to the 2016 Census long-form questionnaire were assigned estimates of exposure to residential greenness based on the mean Normalized Difference Vegetation Index (NDVI) (from 2012 or the most recent year available) within a 500 m buffer around their home, based on postal code. Census weights were used to determine differences in average exposure to greenness according to selected demographic and socioeconomic characteristics.

RESULTS: Mean residential greenness among the 5.3 million census respondents in urban Canada was 0.44 units of the NDVI (standard deviation = 0.18 units). Greenness was lower among immigrants (particularly recent immigrants), some groups designated as visible minorities (particularly people of Filipino ancestry), lower-income households and tenants (i.e., NDVI values ranging from 0.40 to 0.43 units). Greenness values were highest among White non-immigrants and higher-income households (i.e., NDVI values ranging from 0.46 to 0.47 units).

DISCUSSION: Given the potentially multifaceted role that greenness plays in health outcomes, the inequalities in residential greenness described here may contribute to producing or exacerbating existing health inequalities in the Canadian population.

Thermal Transfer on Splinted Implants During Diode Laser Irradiation In Vitro

ABSTRACT

Objective: Laser irradiation is being used for treatment of peri-implantitis. Therefore, this study aimed to assess the heat transfer from laser irradiation on an implant-supported, metal-ceramic fixed prosthesis in vitro. Materials and methods: Two titanium implants were placed in artificial type I bone, and after abutment connection, a bridge was fabricated and cemented. A peri-implant 3-wall defect was created around one of the implants. Thermocouples were placed at coronal (T1/T2) and apical (T3/T4) positions of both implants, and the T5 thermocouple was placed in the pontic. An identical setup was created in the type IV artificial bone. Diode laser irradiation (2 W, noninitiated tips, 320-μm fiber, and noncontact) was performed for 60 sec on each experimental model. This experiment was performed separately with 810- and 980-nm lasers for pulsed and continuous modes. ΔT based on the baseline was recorded during irradiation. Statistical analysis was performed with repeated analysis of variance. Results: Across all experimental models, the recorded ΔT (°) values in T2, T4, and T5 at 10, 30, and 60 sec were significantly less than 10°C (p < 0.001) for both types of bones. For both types of bones, there was a statistically significant ΔT greater than 10°C (p < 0.001) for continuous and pulsed 980-nm irradiation and continuous 810-nm diode laser irradiation after 60 sec. For both 810- and 980-nm lasers, there was a statistically significant ΔT greater than 10°C (p < 0.001) for type I and IV bones under irradiation and only for the type I bone model under pulsed irradiation after 60 sec. Conclusions: Within the limitations of this study, 810- and 980-nm lasers on splinted implants placed in vitro may increase the temperature on adjacent splinted implants due to material conductivity and splinting. Clinicians are advised to keep the temperature lower than the critical threshold following recommended power settings to avoid excessive heat and control complications due to overheating.

Associations between demographic characteristics, perceived threat, perceived stress, coping responses and adherence to COVID-19 prevention measures among Chinese healthcare students

ABSTRACT

AIM: To investigate the associations between demographic characteristics, perceived threat, perceived stress, coping responses and adherence to COVID-19 prevention measures in Chinese Healthcare students.

DESIGN: A cross-sectional survey collecting data in Hong Kong and Fujian Province of China in April 2020.

METHODS: A convenience and snowball sample of 2706 students aged 18 years or older and studying a healthcare programme were recruited in tertiary education institutions/universities in Hong Kong and Putian. The participants completed the questionnaire with six scales: Social Distancing Scale; Personal Hygiene Scale; Empathic Responding Scale; Wishful Thinking Scale; Perceived Stress Scale and Perceived Threat Scale. Path analysis was performed to identify factors associated with the preventive measures outcomes. p value < .05 was considered as statistical significance.

RESULTS: The participants reported high compliances to both social distancing (SoD) and personal hygiene measures (PHM). Confidence to manage the current situation, wishful thinking and empathetic responding directly predicted compliance with SoD and PHM. The final model constructed demonstrated a very good fit to the data.

CONCLUSION: The findings suggest that students who are male, habituate in Hong Kong, have more clinical experience and weak confidence to manage the threat tend to have lower compliance with the COVID-19 preventive measures.

IMPACT: The predictive model constructed is the first one to explore factors associating with the compliance with infection control measures in healthcare students amid the COVID-19 outbreak. As the infection control behaviours of healthcare students, whom are still under training and are the high-risk group of being infected and infecting others in the community, are rarely reported in literature, this study has provided empirical evidence to nurses and other healthcare professionals to identify students susceptible to poor compliance and provide early monitoring and education to suppress the COVID-19 transmission.

Design and Mechanical Evaluation of Sutureless Implants for the Surgery Treatment of Rotator Cuff Tears

ABSTRACT

Rotator cuff (RC) tears cause pain and functional disability of the shoulder. Despite advances in suture anchors, there are still reports about the incidence of surgical-related injuries to RC mainly associated with sutures. The purpose of this study was to design and evaluate the mechanical behavior of sutureless implants to repair RC tears. We hypothesized that the implants present mechanical characteristics suitable for the surgical treatment of RC tears as suture anchors. Three different implants (T1,T2,T3) were designed and fabricated with titanium: T1 has two rods and rectangular head; T2 has two rods with a small opening and enlarged rectangular head and T3 has three rods and a circular head. The implants were fixed in rigid polyurethane foam blocks by a series of blows, and the applied mechanical loads along with the number of blows were quantified. Pullout tests using tapes fixed between the implant head and testing machine grip were conducted until implant failure. The maximum pullout strength and displacement of the implant relative to the rigid foam block were computed. Statistical significance was set at p < 0.05. Owing to its geometric configuration, implant T2 presented the best characteristics related to stability, strength, and ease of insertion. Implant T2 confirms our hypothesis that its mechanical behavior is compatible with that of suture anchors which could lead to the reduction of RC repair failures and simplify the arthroscopic procedure.

Phylogeographic analysis of foot-and-mouth disease virus serotype O dispersal and associated drivers in East Africa

ABSTRACT

The continued endemicity of foot and mouth disease virus (FMDV) in East Africa has significant implications for livestock production and poverty reduction, yet its complex epidemiology in endemic settings remains poorly understood. Identifying FMDV dispersal routes and drivers of transmission is key to improved control strategies. Environmental heterogeneity and anthropogenic drivers (e.g. demand for animal products) can impact viral spread by influencing host movements. Here, we utilized FMDV serotype O VP1 genetic sequences and corresponding spatiotemporal data in order to (1) infer the recent dispersal history and (2) investigate the impact of external factors (cattle density, human population density, proximity to livestock markets, and drought) on dispersal velocity, location, and direction of FMDV serotype O in East Africa. We identified statistical evidence of long-distance transmission events, and we found that FMDV serotype O tends to remain circulating in areas of high cattle density, high human population density, and in close proximity to livestock markets. The latter two findings highlight the influence of anthropogenic factors on FMDV serotype O spread in this region. These findings contribute to the understanding of FMDV epidemiology in East Africa and can help guide improved control measures.
