Effects of acute pain medications on posttraumatic stress symptoms in early aftermath of trauma

ABSTRACT

Posttraumatic stress symptoms (PTSS) develop as sequelae from traumatic injuries. Limited studies suggest that using opioids to reduce acute pain immediately after trauma may also reduce subsequent PTSS, but other pain medications rarely have been examined for preventing acute PTSS. The current study examined the effects of commonly used pain medications, opioid and nonsteroidal anti-inflammatory drugs (NSAIDs), on PTSS after acute traumatic injuries. Participants (n = 71) were categorized into opioid or NSAID group according to their medical records and self-reported medication use. Their PTSS were assessed using posttraumatic stress disorder checklist twice within 2 weeks after trauma. Participants’ pain levels reduced from pretreatment to follow-up in both groups, F(1, 55) = 6.696, P = 0.012, partial η2 = 0.109. Interestingly, a significant interaction between time and medication group on PTSS reached statistical significance, F(1, 69) = 6.014, P = 0.017, partial η2 = 0.080. Follow-up analyses revealed that this interaction was driven by a significant PTSS reduction only in opioid but not in NSAID group. These findings suggested that pain reduction alone is not sufficient to reduce acute PTSS in the NSAID group, highlighting the need to continue further investigations into the mechanisms by which opioids reduce PTSS in the early posttrauma period.

Spanish Validation of the Assessment of Recovery Capital Scale in Clinical Population with Alcohol Use Disorder

ABSTRACT

Recovery from alcohol use disorder involves achieving certain resources for positive lifestyle changes, well-being, and long-term abstinence. The present study aims to translate and validate the Assessment Capital Recovery (ARC) in a Spanish clinical sample of individuals with alcohol use disorder, in abstinence. The participants were 184 patients who attended outpatient treatments. They were evaluated with the adapted version of the ARC (Spanish abbreviation: “Valoración del Capital de Recuperación, VCR”) and by WHOQOL-BREF (quality of life scale), in one session. Statistical analysis included the calculation of reliability, convergent validity (relationship with WHOQOL-BREF), specificity and sensitivity, as well as validity based on internal structure (confirmatory factor analysis). VCR scores show appropriate values for reliability (α = .90), and a low convergent validity with WHOQOL-BREF (Rho = .33-.53). The VCR appears to distinguish between patients with early and stable sobriety (χ2 = 20.55, p < .01). The ROC curve indicates significant discrimination values (p < .05) for stable recovery (5 years of abstinence) and sensitivity of 85.2% and specificity of 71.2%. Further, confirmatory factor analysis suggests the presence of a single factor, with relatively acceptable values of goodness of fit and factor loadings. We used ULS parameter estimation to study VCR properties, an appropriate tool for assessing recovery in clinical populations of individuals with alcohol use disorder in abstinence.

The Chinese Parkinson’s Disease Registry (CPDR): Study Design and Baseline Patient Characteristics

ABSTRACT

BACKGROUND: There is a lack of large multicenter Parkinson’s disease (PD) cohort studies and limited data on the natural history of PD in China.

OBJECTIVES: The objective of this study was to launch the Chinese Parkinson’s Disease Registry (CPDR) and to report its protocol, cross-sectional baseline data, and prospects for a comprehensive observational, longitudinal, multicenter study.

METHODS: The CPDR recruited PD patients from 19 clinical sites across China between January 2018 and December 2020. Clinical data were collected prospectively using at least 17 core assessment scales. Patients were followed up for clinical outcomes through face-to-face interviews biennially.

RESULTS: We launched the CPDR in China based on the Parkinson’s Disease & Movement Disorders Multicenter Database and Collaborative Network (PD-MDCNC). A total of 3148 PD patients were enrolled comprising 1623 men (51.6%) and 1525 women (48.4%). The proportions of early-onset PD (EOPD, age at onset ≤50 years) and late-onset PD (LOPD) were 897 (28.5%) and 2251 (71.5%), respectively. Stratification by age at onset showed that EOPD manifested milder motor and nonmotor phenotypes and was related to increased probability of dyskinesia. Comparison across genders suggested a slightly older average age at PD onset, milder motor symptoms, and a higher rate of developing levodopa-induced dyskinesias in women.

CONCLUSIONS: The CPDR is one of the largest multicenter, observational, longitudinal, and natural history studies of PD in China. It offers an opportunity to expand the understanding of clinical features, genetic, imaging, and biological markers of PD progression. © 2022 International Parkinson and Movement Disorder Society.

Photocatalytic and biological activities of green synthesized SnO2 nanoparticles using Chlorella vulgaris

ABSTRACT

AIMS: To produce tin oxide (SnO2 ) nanoparticles (NP) with microalga for use in azo dye polluted wastewater treatment and to optimize the conditions to synthesize as small NPs as possible.

METHODS AND RESULTS: The green microalga Chlorella vulgaris mediated NPs were synthesized after an optimization process utilizing the statistical response surface methodology (RSM). The optimized synthesis conditions were 200 W microwave power, 0.5 mM SnCl2 concentration, and 200 °C calcination temperature. Methyl orange (MO) was studied for its photocatalytic degradation with UV. Antibacterial activity against four pathogenic bacteria was studied using the well diffusion method. Cytotoxicity was measured using the MMT assay with lung cancer cell line A549, and antioxidant activity using DPPH radical scavenging. Following the optimization of their production, the produced crystalline SnO2 NPs were on average 32.2 nm (by XRD) with a hydrodynamic size of 52.5 nm (by LDS). Photocatalytic degradation of MO under UV was nearly complete (94% removal) after 90 min and the particles could be reused for 5 cycles retaining 80% activity. The particles had antibacterial activity towards all five tested bacterial pathogens with the minimum inhibitory concentrations ranging from 22 to 36 μg/ml. The minimum bactericidal NP concentration varied between 83 and 136 μg/ml. Antioxidant activity was concentration dependent. A cytotoxicity was determined towards A549 cells with an LD50 of 188 μg/ml after 24 h of incubation, a concentration that is much higher than the active concentration for dye removal ranging from 22 to 36 μg/ml.

CONCLUSIONS: After optimization, SnO2 nanoparticles produced with C. vulgaris displayed high photocatalytic activity at concentrations below their antibacterial and cytotoxic activity.

SIGNIFICANCE AND IMPACT OF THE STUDY: The SnO2 nanoparticles produced with the help of microalgae are suitable for the removal of MO dye from wastewater. Further applications of this green technology can be expected.

The Australian and New Zealand dietetics graduate outcomes survey: A cross-sectional study

ABSTRACT

AIM: There is a paucity of comprehensive and current employment data for dietitians. It is unclear if, and where, dietetics graduates are being employed and if they are sufficiently prepared to meet workforce and community needs. The aim of this study was to identify employment outcomes for recent dietetics graduates from Australia and New Zealand at 4-6 months post degree completion.

METHODS: A cross-sectional survey was distributed to graduates from accredited dietetics degrees across Australian and New Zealand. Those who completed their university study in 2020 were eligible to participate. Descriptive statistics and frequencies were analysed and open text answers underwent summative content analysis.

RESULTS: A total of 294 usable survey responses were received from 631 eligible graduates (response rate 47%). Seventy-five percent of graduates were employed in any field and 60% were employed in a role that required their degree. The most common area of dietetics employment was private practice, followed by hospital practice. Most commonly, graduates who did not need their degree to get their job were employed in healthcare-related roles, or nutrition- and food-related roles.

CONCLUSIONS: These findings are important for curricula development to ensure that students are suitably prepared for available employment opportunities. Work-integrated learning experiences may need to shift focus to skill development, rather than a specific job role, to better prepare graduates for the jobs of the future. Further monitoring of employment outcomes is necessary to track changes over time.

Ocular aberrations in eyes with Primary Congenital Glaucoma

ABSTRACT

PURPOSE: To highlight the magnitude of ocular higher order aberrations (HOA) and lower order aberrations (LOA), including component contributions from corneal and internal planes in Primary Congenital Glaucoma (PCG) patients.

METHODS: Consecutive treated PCG patients co-operative for ocular examination and aberrometry, were enrolled over two years for this cross-sectional, comparative, single centre, unmasked study. Best corrected visual acuity, refraction, IOP, wavefront aberrometry and topography (iTrace) were performed and results were compared with unaffected fellow eyes of unilateral glaucoma patients as well as age and sex matched controls with no ocular anomalies other than treatable refractive error.

RESULTS: Both eyes of 32 consecutive PCG patients (17 unilateral, 15 bilateral) and 39 controls were enrolled. The median LogMAR corrected distance visual acuity of PCG eyes was 0.68 (IQR: 0.2 – 1.8). Total ocular (Root mean square (RMS) 1.7µm vs 0.3µm, p = 0.014), corneal (RMS 1.1µm vs 0.3µm, p = 0.004) and internal (RMS 1.1µm vs 0.2µm, p = 0.013) aberrations, as well as HOAs and LOAs at each plane were significantly higher in PCG eyes than in controls. Component HOAs from corneal and internal planes were positively correlated with each other (p < 0.001; rs: 0.7). Total aberrations were greater in the affected eyes of PCG compared to the rest. The predominant subtype of HOAs in PCG were coma and trefoil. PCG with corneal opacity/Haab’s striae had significantly higher astigmatism than the affected eyes with clear corneae at corneal plane (p = 0.02). The aberrations were not statistically associated with the corneal diameter or refractive error in PCG eyes.

CONCLUSIONS: Significantly greater aberrations (Total, HOAs and LOAs, at corneal as well as internal plane) were seen among eyes affected with PCG. Though the exact impact of these aberrations on the final visual outcome is difficult to determine, these could play a pertinent role in compromising visual function, thus impacting the management of visual rehabilitation in these patients.

Analysis of Cancer Knowledge, Attitudes, and Practices in Adolescents and Young Adults in Two Texas Rural Communities

ABSTRACT

The Youth and Young Adults Cancer Knowledge Attitudes and Practices (C-KAP) exploratory study in 2 rural underserved areas in a border community. C-KAP is an interdisciplinary research pilot project led by university scholars in psychology and social work in partnership with community partners. The exploratory cross-sectional mix-method study recruited 141 (n=141) youth and young adults (ages 18-39). This study was informed on empirical research and a bilingual online questionnaire was field-tested, and data was collected via QuestionPro Software. Quantitative analysis was conducted using SPSS version 27. Descriptive statistics and frequency analysis were used for demographics and basic statistics. Chi square tests and Fisher’s exact tests between variables were ran to find statistically significant associations. For the qualitative data, independent coders conducted recurrent content analysis to identify themes. Salient themes include knowledge about cancer types; access to health care; prevention; and the perceived impact of COVID-19 pandemic. Findings highlight a lack of knowledge and orientation on cancer in youth and young adults suggesting the need for community tailored education and screening interventions. Other findings reflect gender differences in knowledge and practices, which indicates that a gender-specific lens is needed when delivering education.

COVID-19 Pandemic Lockdowns Impact on Visual Acuity of Neovascular AMD Patients: A Large cohort

ABSTRACT

PURPOSE: Evaluating the impact of delayed care, secondary to COVID-19 pandemic lockdowns, on visual acuity in previously treated neovascular AMD (nAMD) patients.

METHODS: A multi-center, retrospective, study of nAMD patients previously treated with anti-VEGF injections who were followed-up during 2019 (pre COVID-19) and compared to nAMD patients during 2020 (COVID-19).

RESULTS: A total of 1,192 nAMD patients with a mean age of 81.5 years met the inclusion criteria. Of these, 850 patients were assessed in 2019 (pre COVID-19) and 630 patients assessed in 2020 (COVID-19). 308 patients were assessed through both 2019-2020 and thus were included in both cohorts. There was no significant difference between the years 2020 and 2019 in terms of baseline and change in BCVA (p=0.342, p=0.911 respectively). Mean number of anti-VEGF injections was significantly lower (5.55 Vs 6.13, p<0.01), with constant lower ratio of injections per patient in the COVID-19 period. Baseline BCVA (0.859, p<0.01), number of injections (-0.006, p=0.01) and age (0.003, p<0.01) were predictors of final BCVA.

CONCLUSIONS: In nAMD patients, delayed care secondary to COVID-19 pandemic lockdowns has no statistically significant impact on BCVA. Baseline BCVA, older age and lower number of yearly anti VEGF injections are predictors for decrease BCVA.

Comparative Diagnostic Performance of Estimated Fetal Weight and Isolated Abdominal Circumference for the Detection of Fetal Growth Restriction

ABSTRACT

OBJECTIVES: To describe the comparative incidence, detection of small-for-gestational age (SGA), and composite perinatal morbidity (CPM) associated with diagnostic criteria of fetal growth restriction (FGR) by estimated fetal weight (EFW) <10% with those with isolated abdominal circumference (AC) measurements <10%.

METHODS: We performed a retrospective cohort study of 1587 patients receiving prenatal care and delivery at our institution. We included all patients with ultrasounds and delivery outcomes available, and excluded terminations, second trimester losses, and pregnancies without ultrasounds. EFW was calculated from Hadlock and use of the Duryea centiles, and AC from Hadlock’s reference curves. We determined SGA at birth and defined CPM as birthweight less than 3% or birthweight less than 10% with neonatal morbidity.

RESULTS: Of 1587 patients, 28 (1.8%) were classified as FGR by EFW <10%. Three of 12 patients with isolated AC <10% developed EFW <10% later in pregnancy (25%). The performance of each diagnostic criteria were comparable for the outcomes of SGA and CPM, with similar sensitivities, but with decreased specificity for SGA outcome, and an increased false positive rate for patients classified as FGR by isolated AC <10, with a tradeoff of decreased false negatives.

CONCLUSIONS: Broadening the diagnosis of FGR to include patients with isolated AC <10 did not significantly increase the detection of pregnancies at risk for SGA or CPM. Our conclusions may be limited by a lack of statistical power given a low frequency of SGA and CPM.

Bilateral Transverse Upper Gracilis Flaps for Unilateral Breast Reconstruction: A 4-Year Retrospective Study of the “2-in-1” Technique and a Systematic Review With Meta-analysis

ABSTRACT

INTRODUCTION: The transverse upper gracilis (TUG) flap is a reliable alternative to abdominally based breast reconstruction because of its advantageous anatomy. However, the limited tissue volume prevents reconstruction of large breasts. The bilateral TUG flaps for unilateral breast reconstruction can overcome this problem.

METHODS: A retrospective analysis was performed regarding unilateral breast reconstruction with bilateral TUGs at the Queen Victoria Hospital from 2015 to 2019. Associations between categorical variables were examined with Fisher exact test, and any differences in continuous variables by complication status were evaluated with Mann-Whitney-Wilcoxon test for independent samples. A relevant systematic review was conducted with a meta-analysis of proportions using the Freeman-Tukey arcsine transformation for the estimation of the overall survival rates.

RESULTS: Ten cases were identified with 6 having a delayed reconstruction. Median age and body mass index were 50.8 years and 23.9 kg/m2, respectively, and average flap weight was 268.3 g. One flap was placed superiorly with its pedicle anastomosed to the retrograde internal mammary vessels, except of half of the cases where a second anterograde vein was used. The mean follow-up was 21.4 months, and all flaps survived. Two patients returned to theater, and 4 experienced donor-site problems. The presence of complications was not associated with age, ever smoking, body mass index, TUG weight, or anastomosis to retrograde internal mammary vein. The systematic review resulted in 8 eligible articles with a total sample of 156 flaps. The overall survival rate was 96.8%. In 6 cases, the indication was Poland syndrome (7.7%), and in 48.7%, axillary vessels were also used as recipient vessels. The estimated overall flap survival rate was 100% (95% confidence interval, 99%-100%), and no significant heterogeneity was noted (I2 = 0%, P = 0.71).

CONCLUSIONS: The “2-in-1” technique can lead to symmetrical aesthetically pleasant results for both recipient and donor sites. Our case series and the conducted systematic review reiterate the reliability of this approach.
